I checked all maturity levels as described in bug 701 description and comment #7
instructions.  Only "MATURITY_RELEASE" is not defined.  See PHP warning below. 
I used for that "sample_basic_plainbuttons.phtml" application.

as per discussion with Darren, the MATURITY_RELEASE level was divided into
MATURITY_TECHNICALRELEASE and MATURITY_PRODUCTRELEASE.
