The mini zoom in button for the DHTMLKeymap2 requires that the ZoomIn widget be placed in the application. Although most web applications would have a zoomin button for the main map, I am not sure it the DHTMLKeymap should have this kind of dependency. If for some reason this relationship must be maintained than there should be an error that reflects this issue. Currently the mini zoomin keymap button simply does not work.
this is definitely a bug, probably never found because we don't have apps that don't have a zoom in button. Reassigning to Bill to fix, since he is currently the most knowlegdeable about this widget :>

this works for me, tested on winxp with mapscript_43.dll (okay, it didn't quite work, mapserver 4.3 has introduced more restrictive checking on map extents and the keymapdhtml widget was setting the extents without checking if the min values were less than the max values). I used sample_enhanced.html with the zoomin widget removed (from the nav_basic.html file).
Changed target for verification to 1.99 RC 1.
